The Disaster issue.. Gotta say, I'm quite proud (in a wierd kind of way) to be part of this one. If I recall correctly, This issue came about after a few conversations at MACNA where a few of us were discussing tank disasters and we thought it about time some real life writeups were done, with the aim of showing that it can happen to the best of us at times despite all our efforts and sometiomes long term experience. what you can learn from it, and how you go onwards from there.
